The Portage, WI Senior Living facility includes assisted living, skilled nursing, sub acute rehabilitation and a town center. The goal of the facility is to serve the residents by providing a continuum of care that focuses on restoring residents to their highest level of functioning for discharge / to the level of care most appropriate for quality of life.
The facility is designed to offer a residential long-term living environment that allows residents to remain connected to the community and receive caring help in a dignified way. To this end – the complex includes a town center – with a café, spa, sports room, children’s play area, bank, lending library, meeting/gathering rooms, a chapel/theater as well as out-patient physical therapy services.
Each room is equipped with its own kitchenette and full bath. Rooms are grouped in “households” of 10-15 rooms to promote a sense of community and companionship and create an environment that promotes healing and restoration. Common areas are flexible and centralized. Outdoor and indoor activity areas are provided for residents, with the appropriate level of security for each population.
